Санаторно - курортное лечение беременным женщинам из малоимущих молодых семей.

Правом на получение путевки на санаторно-курортное лечение могут воспользоваться беременные женщины из малоимущих молодых семей (возраст одного из родителей в которой не превышает 35 лет включительно) и постоянно проживающие на территории Липецкой области.

Необходимые документы:

  • Документ, удостоверяющий личность.
  • Свидетельство о рождении/заключении брака.
  • Медицинская справка из женской консультации для получения путевки на санаторно-курортное лечение.     
  • Справки о доходах всех членов семьи за три месяца, предшествующих месяцу подачи заявления. Трудовая книжка не работающего члена семьи.

Консультирование граждан осуществляется по бесплатному номеру ЕДС. 88004504848, добавочный 2.

Прием документов производится в филиале и отделах  ОБУ «УМФЦ Липецкой области» по адресам:

г. Липецк, пл. Победы, д. 6а;

г. Липецк,  ул. Меркулова, д. 45;

г. Липецк, ул. Кривенкова, д. 11а;

г. Липецк, ул. 40 лет Октября, д. 25;

ул. Коммунистическая, д.20 (в помещении Городского Дворца культуры).

В соответствии со статьей 9 закона Липецкой области от 27.03.2009 № 259-ОЗ «О социальных, поощрительных выплатах и мерах социальной поддержки в сфере семейной и демографической политики, а также лицам, имеющим особые заслуги перед Российской Федерацией и Липецкой областью» предоставляются путевки на санаторно-курортное лечение беременным женщинам из малоимущих молодых семей.
