Подать заявление на соцконтракт теперь можно удаленно.

Подать предварительное заявление на получение помощи на основании социального контракта теперь можно по электронной почте.

Для этого необходимо отправить заполненное заявление на адрес sockontrakt@admlr.lipetsk.ru. Через неделю после получения заявления центром соцзащиты заявителю сообщают: подходит ли его ситуация под условия предоставления помощи на основании соцконтракта или нет.

Если проходит, то заявителя пригласят в определенный день с оригиналами документов:
*паспорта заявителя и членов семьи старше 14 лет;
*справки о доходах заявителя и членов его семьи за три месяца предшествующих месяцу обращения за соцконтрактом;
*документ подтверждающий родственные отношения заявителя и членов его семьи;
*при наличии предоставляются медсправки о медосмотрах, диспансеризации, сертификат о вакцинации.
