Социальные путёвки в оздоровительные лагеря.

Предоставляются для детей от 7 до 15 лет включительно:

- находящимся в трудной жизненной ситуации;

- оставшимся без попечения родителей;

- детям-инвалидам;

- жертвам вооружённых и межнациональных конфликтов, экологических и техногенных катастроф, стихийных бедствий;

- детям из семей беженцев и вынужденных переселенцев;

- детям, проживающим в малоимущих семьях;

- детям из многодетных семей;

- сиротам.

 

Прием документов производиться исключительно по предварительной записи по тел.: 27-50-70  по адресу: г. Липецк, ул. Гагарина, д. 26.

 

Предоставляются следующие документы:

- заявление;

- документ, удостоверяющий личность заявителя;

- свидетельство о рождении ребёнка;

- полис обязательного медицинского страхования (ОМС) ребёнка.

 

Внимание! Открытие оздоровительных детских лагерей зависит от эпидемиологической ситуации в Липецкой области.
